Welcome to MRS SCOTT’S WHERE HEART IS HOME, a compassionate and intimate 6-bed Residential Care Facility for the Elderly (RCFE) located at 292 W Trenton Ave in the tranquil city of Clovis, California. Our small-scale community provides a secure, home-like environment where seniors can receive personalized assistance while maintaining independence and quality of life. We fully comply with all California-required basic care services for RCFEs, including assistance with activities of daily living, medication management, nutritious meals, housekeeping and laundry, and individualized care plans to promote health and safety.

In addition to our core services, we offer these specialized amenities and features based on facility details:

• 24-Hour Supervision: Continuous oversight by trained staff, with a licensed R.N. on call 24 hours daily for medical support.
• Assistance with Daily Activities: Help with bathing, dressing, grooming, transfers, mobility, and incontinence care.
• Medication Management: Coordination and administration of medications, including coordination with health care providers.
• Hospice and Palliative Care: Facilitated hospice care for end-of-life support.
• Memory Care Support: Care for residents with Alzheimer’s or Dementia.
• Non-Ambulatory Care: Support for wheelchair-bound or non-ambulatory residents.
• Meal Preparation and Service: Nutritious, home-cooked meals tailored to dietary needs and special restrictions, served in a communal dining room.
• Housekeeping and Laundry: Regular services, including dry cleaning.

• Additional Amenities: High-speed internet, newspaper delivery, on-site beautician, emergency alert system, move-in coordination, and furnished private or semi-private rooms (options for 1-bedroom, studio, or 2-bedroom accommodations).

The program begins with a comprehensive pre-placement assessment. Based on our assessment, we develop a personalized plan of care designed to meet your loved one’s unique needs and provide them with the quality of life they deserve.
