Vi at Palo Alto Care Center (Continuing Care Retirement Community – CCRC)

Vi at Palo Alto is a Type A Continuing Care Retirement Community (CCRC), also known as a Life Plan Community. This means that independent living residents have access to a continuum of care in our supportive residential setting, so they can receive assisted living, skilled nursing, or memory care services as the need arises.

Vi at Palo Alto’s Care Center also offers care services to non-CCRC residents as current occupancy and admission criteria allow. Our skilled nursing setting is also Medicare-certified. We accept Private Pay, Traditional Medicare (Parts A & B), and where the circumstances allow, we can process payment from other participating third-party or private insurance coverages.

Assisted Living at Vi at Palo Alto

There may come a time when you need extra help with daily living.

Our care center offers that help, with an emphasis on maintaining dignity and privacy while promoting an ongoing sense of independence and well-being. Our assisted living residence is staffed 24 hours a day, seven days a week, by a team of dedicated caregivers under the supervision of a licensed nurse. And because our care center is on site, you’ll never be far from the community of friends and family you love.

Assisted living lifestyle

We’ve created an environment designed to help residents embrace everything life has to offer: a variety of menu options from our classically trained culinary team, a stylish dining room, weekly lifestyle programming, and scheduled transportation within 15 miles for shopping trips and special events.

Services provided in assisted living

Depending on how much help you need, our care providers can assist with tasks from medication management and administration to bathing, grooming and dressing. Vi at Palo Alto also provides weekly light housekeeping service and laundering of bed and bath linens.

Additionally, we can arrange for a variety of services*:

• Pharmaceutical services
• Private duty assistant services
• Transportation and dry cleaning
• Physical, occupational and speech therapy
• Ancillary medical supplies and services
• Salon and barbershop services
• Guest meals (advance reservations requested)

Skilled Nursing at Vi at Palo Alto

Whether you’re recovering from surgery, in need of rehabilitation services or require long-term nursing care, Vi at Palo Alto’s dedicated skilled nursing staff is ready to help. Our team of licensed professionals manage care plans focused on each resident’s individual needs, including physical, speech and occupational therapy. And because our care center is on site, you’ll never be far from the community of friends and family you love.

Person-centered care

Our care center has therapy teams licensed to work in many treatment modalities for residents with a multitude of clinical, occupational and cognitive needs. We encourage collaboration between residents’ physicians and community staff and ensure that medical orders and treatment plans are successfully carried out by our interdisciplinary team.

Skilled nursing lifestyle

We’ve created an environment designed to help residents embrace everything life has to offer: a variety of menu options from our classically trained culinary team — served in a stylish dining room or in residents’ rooms — weekly lifestyle programming, and scheduled transportation within 15 miles for appointments, shopping trips and special events.

Services provided in skilled nursing

We create an individualized care plan for each resident after a comprehensive baseline assessment, which may include:

• Assistance with daily living activities, including bathing, oral care, grooming, dressing and eating
• Physical, occupational and speech therapy
• Medication administration and monitoring
• Wound management and skin care
• Hydration and nutrition monitoring
• Coordination for hospice care

Vi at Palo Alto’s memory support unit offers a comfortable, structured environment designed to enhance the social, emotional and physical well-being of residents with mild to moderate Alzheimer’s and related cognitive conditions. Canvas by Vi, our hallmark memory support program, provides care that treats every resident not just as a patient, but as a whole person. Our compassionate approach helps each resident live their most remarkable retirement in a way that meets their personal and independent needs.

What to expect in memory support

Our memory support staff meets with family members and creates a customized care plan with specialized programs and services designed to meet each resident’s unique needs. In addition to this person-centered care approach, we offer a range of services and amenities, including daily housekeeping service and laundering of bed and bath linens.

Even meal time is designed to be easier for memory support residents, with carefully selected, easy-to-handle menu items and tablecloths, napkins and tableware in contrasting colors to decrease confusion. Plus, all meals are prepared with care under the supervision of our executive chef.

We offer a variety of activities designed to engage memory support residents:

• Walking, dancing and structured exercises to maintain muscle strength and flexibility
• Montessori Method–based activities to support motor skills and concentration, as well as build self-esteem

Support for families

Vi at Palo Alto helps families with educational resources to help them learn about dementia and how it will affect their loved ones. Our care center can also refer families to area support groups.

• Home maintenance and repairs: Our on-site staff takes care of your home with regularly scheduled maintenance, along with any home repairs you may need.
• Lawn care and landscaping: We have a dedicated team to meticulously maintain the community’s grounds, complete with beautiful California plants and a community rose garden.
• Housekeeping: Your list of chores just got a little shorter: weekly housekeeping and laundry service for your personal linens is included for residents.
• 24-hour concierge: Our front-desk concierge is always on duty to help out, answer residents’ questions and offer a friendly welcome to everyone who walks through our doors.
• Free valet parking: Come and go freely, 24 hours a day. Valet parking in our heated garage is free to you and your guests — no tipping, please!
• Scheduled transportation: Need a lift to the grocery store or doctor’s appointment? Schedule transportation within 10 miles to take care of your errands, no driving needed!
• Building maintenance: Our maintenance staff keeps common areas and building exteriors in top condition, so you can enjoy your days without the ongoing chores of home ownership.
