Person-Centered Programming

Every Memory Care resident at Cascade Park Gardens receives a personalized care plan tailored to them. Our compassionate staff works closely with you and your loved one to create this living document, noting your loved one’s unique qualities, abilities, interests, and preferences, and guiding their day-to-day experience.

We are more than just the bullet points you see below, but here we hope you’ll find some answers to what is included in our basic rates. We provide these services as an unobtrusive friend to ensure your basic needs are met. We are there when you need us, and remain in the background when you don’t.

Services

We provide these services to all residents:

• Weekly housekeeping services
• Weekly linen and laundry services and as-needed
• Maintenance services
• 3 meals daily + healthy snack choices that follows a liberalized diet overseen by a dietician.
• Scheduling doctor appointments and coordinating transportation.
• Nursing and caregiver services to include Activity of Daily Living needs. We can manage diabetes, medications, vital signs, assistance with dressing, showering, mobility, and more.
• Therapy Services: We have a home health provider that can provide PT and OT services along with our Adult Day Health program.

Amenities

Our Amenities are curbed around using our space effectively for resident interests and needs. Activities that focus on socialization, laughter, and movement occur throughout the community. We also have a calendar of events that changes monthly to celebrate birthdays, changing seasons, holidays, and our residents.

In-House Specialty Services: The Wellness Advantage

When we accept a new resident to move in we are first looking to see if we can honor them with quality care. With 12-16 hours of nurses on staff every day and incredible wrap around services which include on-site: Home Health, PT and OT, weekly primary care doctor visits, nurse practitioner, dental care, foot care, and more. Our programs support aging in place for as long as possible until safety demands otherwise.

Expert Care

Our expert nurses provide clinical oversight of blood sugars, insulin injections, vitamin B injections, lab work, keeping the many primary care doctors informed, and consulting with them for care services. Our nurses also consult with caregivers, provide support for caregiving needs, watch for changes in a resident’s normal condition, and write personalized care plans.

Licensed Nursing

We are committed to providing 12-16 hours of on-site licensed nursing staff each day that oversee resident care. Our Wellness Team has fostered community partnerships to bring in dental hygienists, home health agencies with a PT/OT office onsite, regular podiatry appointments, regular primary care doctor visits, nurse practitioners onsite, and daily pharmacy deliveries.

Horticulture and Pet Therapy

Natural Healing

Studies have confirmed that nature is the best antidepressant available. While we reside within walking distance of the Tacoma Mall, we facilitate excursions and opportunities to dig fingers into dirt, smell old growth cedar trees, and watch the clouds at places like Point Defiance Park.

Visiting Animals

Dogs remain our best friends and provide unconditional love. We have lovely community dogs that visit and other pets, including goats, sheep, miniature ponies, and rabbits. The love of an animal provides incredible therapies and vital emotional contact.

Clinical Services

Cascade Park Gardens Memory Care’s personal care program was developed for residents needing assistance with activities of daily living. We can provide assistance with ambulation, dressing, grooming, toileting, bathing, medication assistance, injection administration, diabetic care, personal laundry service, and general health care monitoring.

Plan of Care

Within 14-days of the resident’s move into our community, our clinical staff will complete a plan of care that addresses the resident’s assessed health care needs, social needs and preferences, personal care tasks, and if applicable, limited nursing and medication services. Each resident has the right to be fully informed about his or her negotiated service plan.

Care Plan Updates

Each resident of Cascade Park Gardens Memory Care will have their negotiated service plan reviewed at least annually and/or with a change of condition. Residents and their families are encouraged to participate in this planning process.
