Walnut Creek Senior Living Campus

Our clustered suites encourage community and neighborliness. Each is designed for comfort, convenience, and safety and can be private or shared by two people. Residents can decorate with personal touches and furnishings or use provided pieces.

Suites include a full bathroom with walk-in shower, wall-to-wall carpet, refrigerator, pantry, safety features, a 24-hour emergency response system, and private French doors leading to the outdoor patio or courtyard. Amenities include a hair salon, personal laundry, wellness clinic for visiting physicians, library, Ice Cream Parlor, and more.

Walnut Creek is currently certified by the Ohio Department of Aging to provide the basic service under the Assisted Living Waiver program. We are currently accepting individuals who are enrolling in the Assisted Living Waiver program.

Rehabilitation

For residents requiring intensive rehabilitative care, an array of resident-centered therapy programs are coordinated by the facility’s therapy team in our on-site therapy unit, gymnasium, and occupational therapy apartment. When therapy goals are achieved, staff offers an at-home evaluation to assist in a smooth transition.

Memory Care

Hospice Care

Walnut Creek contracts with several hospice agencies throughout the Miami Valley, giving families multiple options for care. Residents receiving hospice services may be eligible for enhanced support including:
