Bridges at The Shores (Hospice and Palliative Care)

Skilled Nursing

United Methodist Communities at The Shores provides around-the-clock skilled nursing (also known as long-term care) for frail or medically complex residents. The continuum of care allows for a graceful transition from assisted living within the same community.

Some common conditions appropriate for skilled nursing include post-operative wounds, stroke, injuries, diabetes, infections, Alzheimer’s, cardiovascular ailments, Parkinson’s disease, dementia, or respiratory ailments. Our team of licensed nurses, certified aides and therapists deliver this skilled nursing, broadly referred to as long-term care, which comprises custodial and skilled nursing and medical treatment from licensed nurses 24 hours a day.

Sometimes individuals require skilled nursing on a temporary basis due to an injury or illness; however, it is also appropriate for individuals with medical needs including:

Rehabilitative Care

Transitions Program

United Methodist Communities recognizes that hospital and rehabilitation patients may not be ready or wish to return home upon discharge. Ideally, rehabilitation should occur in the least restrictive environment to support overall recovery and wellness. Sometimes, going home can limit recovery for various reasons.

Transitions bridges the recovery of such individuals at The Shores. It supports the goal of returning to a home environment for the long term. Transitions’ care and service options support shorter hospital lengths of stay, decrease emergency department use and reduce patient re-admissions.

Transitions’ clients receive the appropriate care, including clinical, psycho-social, emotional and dietitian services; rehabilitation therapies; social engagement; wellness programming; home environment assessments; and transitional care to home with follow-up. Stays usually last between one and six weeks.

Examples among the distinct reasons for a Transitions stay include:

  • Trial use of new or modified medications
  • Practice for living in a home-like environment
  • Anxiety about going home without clinical support
  • Delay in implementing support structures at home
  • More stabilization required

Transitions fully furnished apartments come with dishware, linens and television services, requiring clients to bring only clothing and personal items. They may participate in the dining program, community life offerings, use the common areas and elect additional optional services.
