Permanent Residential Care

After months or years of debating the best care solution for your loved one, you have decided it is time for a residential home—one that provides the quality 24/7 care, safety, support and positive stimulation that your loved one needs and deserves—at a price you can afford. Visit other facilities, then enjoy a tour of The Country Home and spend some time with our management team.

If you join us, The Country Home will assure the highest level of independent functioning and quality of life for your relative. You will have worry-free peace of mind, knowing you made the best possible decision. You may stay as closely involved with your loved one as you like, visiting anytime. Bring any questions, concerns, or suggestions to our administrator or other staff so we can address them immediately.

  • Medication management by certified staff; weekly check-ups by an RN; three nutritious, home-cooked meals and snacks
  • Assistance with bathing, grooming, dressing, eating, walking, toileting, etc.

A team of our staff members—our registered nurse, the admissions manager, the administrator–will obtain all relevant information from the prospective resident, the family and health care providers so we can do a thorough assessment and initial care plan. This usually involves a home or hospital visit. Sometimes the assessment is done at The Country Home.

Prior to admission, new residents must have a two-step TB test, a MRSA test and a flu vaccination, during flu season.

Medical records, other paperwork and medication supply must be fully resolved at least 48 hours before admission.

The family provides transportation, incontinence products and special dietary or health items.

While our rooms are all very tastefully furnished, you may decorate the room as uniquely as you like, to make it resemble the environment your relative enjoyed before joining us.

If a resident has to go temporarily to a hospital, skilled nursing facility or rehabilitation center, family members may arrange to have us hold their room so they can return “home” as soon as possible. Our staff will visit the resident in the other facility and coordinate re-admission with health care providers and family, if they remain appropriate for The Country Home.

If a resident needs supplementary services at The Country Home, such as speech therapy, physical therapy, or podiatry, our staff can help coordinate the services with family and service providers.

Short-Term Stays or “Respite Care”

Short-term residential care—a rest for caregivers and a stimulating social experience, health care, and supervision for your loved one! You can go on vacation, a business trip, or just take a much-needed break from the stress and exhaustion of caregiving. Short stays can also serve as an excellent way to make a gentle transition to permanent residential care.

After a few successful short stays, you and your loved one will both feel “at home” in The Country Home, knowing we are here for you when the time is right.

  • Same admission process and requirements as for a permanent resident.
  • Prior to admission, short-stay residents must have a two-step TB test and a MRSA test and a flu vaccination, during flu season.
  • Short stays are accepted.
  • Like permanent residents, those coming for a short stay enjoy all the services and activities of a permanent resident.
  • Minimum of three, maximum of 30 consecutive days, when available.
  • Medical records, other paperwork and medication supply must be fully resolved at least 48 hours before admission. Depending on the interval between short stays, paperwork and pre-tests might need updating.
  • The family provides transportation, incontinence products and special dietary or health items.
