The Rose Linn community proudly offers a continuum of care. As your loved one’s needs change, we are here to support them. Rose Linn Vintage Place offers memory care and residential care services. Rose Linn Care Center offers extended and skilled (24-hour) nursing care. In both settings, residents feel at home. Our staff believe that caring for your loved one requires not just an understanding of symptoms, but an understanding of people. The Rose Linn campus is conveniently located ½ mile off of I-205 in beautiful West Linn.

At Rose Linn Care Center, we work hard to provide high quality care & attention to your loved ones. And thankfully, it shows! Recently, Rose Linn earned a 5-star rating via CMS (Centers for Medicare & Medicaid Services).The overall rating is based on a facility’s performance from 3 sources: health inspections, staffing, and quality measures. Our diligent efforts over the years have resulted in a rating we are proud of!

Rose Linn’s campus offers various levels of care options in one convenient setting. These include extended/skilled nursing care, memory care, and residential care. We are excited to offer this continuum of care, as it meets the needs of our residents at every part of their aging journey. Our philosophy at Rose Linn is to build on a person’s strengths, therefore, we encourage personal independence.

We find that family members who tour our facility can better determine which level of care best meets the needs of their loved one. Case managers, discharge planners and family members work closely together to coordinate each move-in to Rose Linn. Families quickly discover that we can manage their loved one’s care and service needs seamlessly. We are always prepared to move loved ones to and from the appropriate level of care when needed. Extended/Skilled Nursing Care

Rose Linn Care Center is our skilled nursing facility which provides extended care. This space specializes in dementia care and also suits residents needing end-of-life care. Most importantly, Rose Linn Care Center provides 24-hour supervision and nursing to our residents. Therefore, this portion of our campus provides solutions for people who have disabilities or chronic illnesses, as well as residents diagnosed with late-stage Alzheimer’s.

Memory Care Options

Rose Linn Vintage Place includes a memory care community endorsed by Oregon State’s Department of Human Services. We offer a safe and comfortable home for those with dementia or memory loss. Our tasteful community is monitored by specially trained staff. These professionals seek to know each resident. For instance, we value each individual’s interests and we listen to their stories. Ultimately, we personalize their care to create experiences that are meaningful for them.

Residential Care Options

Residential care at Rose Linn Vintage Place supports residents needing help with every day care. We assist with activities of daily living, such as bathing, grooming, dressing, eating, mobility and medication management. We offer studio, one-bedroom and semiprivate apartments. Our Residential Care community prioritizes socializing and community outings.
