Heatherwood is a memory care facility designed specifically for persons living with Alzheimer’s and Dementia related cognitive challenges.

Despite the challenges of memory impairment, persons with Dementia and Alzheimer’s can, and do, continue to lead fulfilling and purposeful lives. Our memory care community is designed and set up for fun and safe engagement between our residents, staff and guests. Our community provides a safe and secure environment, with the peaceful tranquility of the outdoors.

About us

Heatherwood Assisted Living is a homelike community nestled in the heart of downtown Walnut Creek.

Heatherwood is a privately owned memory care facility dedicated specifically for resident’s who are living with Alzheimer’s and Dementia related cognitive challenges.

Nestled in the heart of downtown Walnut Creek, Heatherwood Memory Care allows for a beautiful home-like setting for our residents as well as a vast array of shopping and exquisite culinary dining options only blocks away for our resident’s, their families and guests.

At Heatherwood we take pride in making the transition into our community as comfortable as possible. We believe that taking the time to understand the needs of our families is truly what sets us apart from other communities. Upon meeting with our experienced staff we will ensure that the process is both informative and effortless.

Here at Heatherwood we offer:

• a full-time activities program 7 days a week
• an individually created nutrition and care plan for each resident
• assistance with medication management
• wandering and fall risk reduction plans

Each of our spacious private and semi-private rooms have direct access to our secure courtyard. Our community offers the tranquility of the outdoors yet provides a safe and secure environment.

Our memory care community is designed and set up for fun and safe engagement between our residents, staff and guests.

Resident Services

Each of our residents requires varying levels of day-to-day support from our staff.

The following is a list of some of the basic services that we provide to make the lives of our residents less stressful and allow them to focus on the enjoyment and fulfillment of life in our community.

Personal Care

Although we promote the independence of our residents, we understand that some of the basic activities of daily living may become more of a challenge over time. We offer assistance with tasks such as: bathing, dressing, grooming, and assistance in traveling to and from daily events.

Residents enjoy housekeeping services daily, as well as weekly laundering services.

Incontinence Care

More than half of all residents in care facilities are incontinent. Taking care of someone with incontinence is a demanding job. From the physical tasks to the psychological nurturing, taking care of another person shows strength, compassion, and tremendous amounts of character.

Our staff is here to provide this invaluable service.

Medication Support

Our licensed nurse communicates and confirms medication orders and changes with each resident’s primary care physicians.

Our staff assists our residents in need with administering their medications, while communicating their effectiveness and changes with the resident’s families and their physicians.

Dining Services

Sharing meals with family and friends is a meaningful part of life. At Heatherwood, we try to make each meal special, whether it’s giving the best service, creating the perfect atmosphere or preparing great food. We know that personal taste preference is just as important as nutrition. Our staff tailors meals to the individual needs and likes of our residents.

We have the ability to accommodate special dietary needs and food restrictions. This includes liquid thickeners, chopped, pureed, and liquid diets.

Support Services

We can help in the planning and scheduling of physician visits, as well as organizing transportation to and from appointments.

We also have general, podiatry and dental care physician’s scheduled on a regular basis to meet the needs of those residents that have difficulties leaving the facility for any such services.

Activities

Daily life at Heatherwood is focused around our inclusive activities program.

Although there are many challenges to face when Dementia and Alzheimer’s disease are diagnosed, it does not stop our loved ones’ desire for purpose and fulfillment.

Our activities program is committed to providing opportunities for social interaction, mental exercise and spiritual fulfillment, which are designed to suit each resident’s preferences.

Our staff focuses on daily moments of success.

Here at Heatherwood, we stress health and wellness, which we emphasize through exercise. Heatherwood residents enjoy professional fitness training twice a week. Residents can also participate in our multitude of programs that take place from morning to night, providing both fun and multi-sensory stimulation.

Our residents can choose from a broad range of activities including: games, painting, musical performances from outside entertainers, or even our mini beauty salon where residents can be pampered and have their hair trimmed and styled.

Our residents are not confined to solely indoor activities. Outside of group activity time we have an interior courtyard and exterior patio area to allow our residents to have time to themselves for personal reflection or just to get some fresh air. Residents are given the option to dine, read, or relax in these secure and comforting area.

Family and friends are welcome to participate in all community activities and events. We know that the responsibilities of every day life may make it difficult to constantly be a part of our residents’ lives. We encourage family and guests to spend whatever time they can with residents. Unlike traditional visiting hours, our guests can enjoy time at their convenience throughout the day and night.

We also recognize the importance of communicating openly and regularly with family members to ensure we are providing the very best service for our residents.
