Harmony Hope Care Home is a licensed adult foster home in a quiet SE Portland neighborhood, led by Rahel Yetbarek, an Executive Director and Certified Nursing Assistant with more than 15 years of caregiving experience, including 6 years running adult foster homes. Rahel brings hands-on expertise in hospice care, medication administration, behavioral support, and caring for adults with complex medical needs and disabilities. She personally oversees each resident’s individualized care plan and trains and supervises staff, so every resident receives attentive, consistent care.

The small, family-style home supports residents with general and moderate care needs, memory care and dementia, and hospice and palliative care. The home is single-level with a ramped, step-free entrance, grab bars in the bathroom, and a large fenced backyard with a covered patio.
