The transition to memory care can be one of a family’s most difficult decisions. At Autumn View Gardens, our memory care services emphasize safety and quality of life. Memory care services include around-the-clock, compassionate care and supervision in a secure environment. We create an individualized program for residents. Additionally, our memory care services are all-inclusive, meaning we don’t charge for additional care services even as a resident’s needs increase.

Support as Needs Progress

With BLOOM, our residents stay active in each Wellness Petal. This can slow the progression of Alzheimer’s or dementia and ease its effects. To help manage anxiety, wandering, and sundowner syndrome, BLOOM trains our Care Partners to personalize their approach to each resident. Additionally, our communities offer various gardening areas, social settings, physical activities, and spiritual programs designed to care for and support individuals with dementia and their families.

Personal Care Plan

Residents meet with our Health Services team to establish a comprehensive personal care plan based on an assessment and in-person meeting. This care plan is updated regularly to ensure the residents’ care needs are always met.
