The Slovene Home Skilled Care & Rehabilitation places quality of care first and is designed to provide the services and support you or your loved one need as you transition either from the hospital or the community. Our services include post-acute hospital care, long-term care, assisted living, hospice and respite care.

As part of the Cleveland community for more than 60 years, the Slovene Home has earned a reputation of providing quality medical care. Our home-like atmosphere allows for compassionate, one-on-one interaction between residents and their team of professionals that includes physicians, nurses, rehabilitation therapists, dietician and social services. Our multidisciplinary approach works with our patients, residents and their family to design physician prescribed, personalized comprehensive care plans to optimize each individuals’ unique needs. We promote desired outcomes and prevent hospital re-admission.

The Slovene Home Skilled Care & Rehabilitation offers a variety of services to assist our patients with 24-hour skilled nursing care. Physical, Occupational and Speech therapies are available seven days a week. Our multidisciplinary team implements physician prescribed individualized comprehensive care plans to optimize each patient’s health. We provide person-centered care and promote wellness, desired outcomes and quality care to prevent hospital readmissions.

We can reserve a private rehabilitation suite upon request before your scheduled procedure. Patients and family members can inquire with admissions to ensure a private suite is available at time of discharge from hospital, or in the event respite care is needed.

We also provide services for patients’ and residents’, families, guardians and responsible parties of our residents. Outreach services for the elderly living in the community-at-large are also offered whenever possible.

In August 2016 we opened our doors to a nine-suite Assisted Living Home, “The Lindens,” within our facility. Residents are provided extra assistance they need in a warm, community atmosphere while maintaining their sense of independence. Our Assisted Living provides around-the-clock personalized nursing care and offers medication reminders, assistance with personal care and provides entertaining social activities and events. The 24/7 availability of care and support grants peace of mind to our residents and families.

The Assisted Living wing offers three suite options. Our deluxe and one-bedroom suites feature kitchenettes, as well as the option to dine on three meals per day designed by our on-staff registered dietician.
