At Caring Hearts, we are a licensed 12-bed Residential Care Facility for the Elderly (RCFE), serving the communities of West San Jose, Campbell, Santa Clara, Saratoga, and Los Gatos. For over a decade, our facility has been providing exceptional care for elderly residents, making us one of the most trusted and respected assisted living homes in the area.

What assisted living options are available?

Caring Hearts offers assistance with various daily living needs. We offer a shared room for women and private rooms for either gender. We have bedridden and hospice waivers in place to assist with those needs when the time comes.

Services
Furnished, Pleasantly Decorated Rooms

Rooms can be furnished by the facility or the family has the option to bring in their own furniture to provide familiar items to aid in the transition. Private rooms can have a twin size bed, night stand, visitors chair, dresser, and closet or armoire. We encourage families to put up family photos on the wall and other decor to provide comfort and familiarity.

Cost includes a furnished or semi-furnished room (bed, night stand, closet/armoire, and dresser). Most residents bring in their own furniture to help the transition by having familiar items in their new surrounding.

Personal Care

We take a proactive approach to the care and supervision we provide to our residents. Our 24-hour awake staff are trained, competent, and loving caregivers who practice flexibility in adjusting and tailoring their care to the needs of the resident.

We make sure to maintain routine communication with family regarding the well-being of their loved one and provide assistance with transportation management. We also offer pre-planned daily activities and special events throughout the year that keep our residents engaged and socially active.

Our care plans are comprehensive, monitoring details to help circumvent avoidable issues or address them before they get too bad. Each resident comes to us with their own particular way of doing things, likes/dislikes, and health care needs so try to tailor their plan to what works best to meet those needs.

Our goal is to create peace of mind for our residents’ families.

Our average ratio is one staff to 3 residents.

Medication Management

We have trained medication aides who handle our medication management. This program includes keeping stock of the inventory, ordering refills, receiving and implementing new medications, and ensuring timely administration. We use pharmacies that deliver to us to help relieve families of the run around. The co-pays are generally the same, even for Kaiser patients. We also provide feedback to the family or the doctor directly for new medications so they can know whether a medication is working effectively.

We are a non-medical facility and therefore do not employ medical nurses or doctors. However, we work closely with them as a team effort to enhance the care plan. We train a few medication aides per the guidelines of the State of California code of regulations, Title 22.

Meal Service with Appetizing Menus

We provide three meals a day and snacks. Breakfast is served as residents arrive to the dining room since they get ready at different times. Lunch and Dinner are generally served as a group. If there is a meal from the menu of the day that the resident doesn’t care for, we can provide a secondary option based on family and resident input that we can keep on hand for cases like this.

We also offer special diets (as prescribed by a physician).

We serve American-based foods with a balance of the food groups.

Housekeeping Service

Each resident’s room is cleaned daily, with a deeper cleaning conducted monthly.

Laundry Service

Individual general laundry is scheduled for once a week and conducted in house. For clothes or bedding that are wet or soiled, we will wash them the same day. We advise for families to put fabric name tags on the clothing for staff reference.

Senior care social nurturing program

As our elderly age, life changes happen right along with them. They may not be able to do what they used to do as physical limitations progress. These limits may begin to bring on a feeling of isolation and loneliness.

Being in the company of others and engaging with them in a variety of ways provides a sense of belonging, a natural human need. With our preplanned activities and training of staff on purposeful engagement, we try to develop new bonds and relationships within the context of our community to enable your loved one to feel a renewed drive and improved quality of life.

Comprehensive senior care plan

At this stage of life, the slightest changes in our health condition can wreak havoc or exacerbate symptoms that would normally be manageable in adults. We strive for a proactive approach and try to monitor areas that cause common health issues for our elderly adults, such as dehydration and constipation that could trigger behavior changes or pains that could be difficult for the elderly to describe or point out.

Our staff provide daily reports so that we are able to identify trends that could mean an illness is underway so that we may communicate the observations to the health care provider in a timely manner. Some issues are asymptomatic but this method gives us a way to keep an eye and work on things that we can before they get worse.

24-hour awake staff

Our typical resident comes from the larger facilities or from home, and require more attention, even through the night. Issues such as getting up in the middle of the night to use the bathroom or getting up out of confusion can lead to falls. During the night we have awake staff to enable us to provide regular dry checks to prevent skin break down, and/or assist in a variety of incidental needs requested by the resident.

Each resident can be provided a wireless call button so they may connect with staff for assistance. Each bed also has a wireless bed mat alarm that triggers our central system to alert staff that a resident is likely getting up so we may get to them as soon as possible.

Visit Our Home

Caring Hearts is a medium sized senior community providing care for up to 12 residents. Our facility is a single story home located in West San Jose, near Campbell.

Room

They can be furnished by us or furnished with your loved one’s personal furniture and decor. New is usually a difficult thing. We support the idea for rooms to be decorated with personal items to help with a smooth transition.

Living Room

We encourage our residents to be outside of their rooms when not napping or sleeping to help keep days/nights on time and to encourage social nurturing.

Dining Room

We offer cafe style seating to help pair the different personalities and needs of our medium size community. We can support special diets needs depending on physician’s orders, such as soft mechanical or puree. Our staffing ratio allows us to help clients with varying degrees of assistance.

Open-air Courtyard

Delightfully landscaped courtyard to provide residents, families, and visitors a comfortable area to enjoy the outdoors. This lovely courtyard has table seating, ample shade, and periods of sunshine for those moments we want to enjoy the warmth.
