Nestled among six acres in San Jose’s Willow Glen neighborhood, Lincoln Glen Manor is dedicated to enhancing the lives of seniors by providing four levels of care.

Conceived in 1965 by the Lincoln Glen Church Congregation, Lincoln Glen Manor was built in 1969. At that time, the community felt a responsibility to care for their seniors. Thus, the concept of Lincoln Glen Manor was instituted as a caring community for seniors adjacent to Lincoln Glen Church. Today, Lincoln Glen Manor provides four levels of care for seniors.

Continuing Care Residence Program

As a California Continuing Care Retirement Community (CCRC), Lincoln Glen offers specialized long-term care including housing and residential services as well as a skilled nursing that supports residents for a lifetime. Residents choose to live in CCRCs for a variety of reasons.

Part independent living, part assisted living and part skilled nursing home, CCRCs offer the range of care that residents need as they age. The CCRC allows residents to move into a new level of care, but continue to live in a comfortable, familiar setting for as long as they need.

The assisted living center

With 30 studio units as well as a one-bedroom suite, there’s no shortage of space to comfortably enjoy retirement years. Each unit is furnished with modern and attractive pieces with residents needs in mind.

• Scheduled transportation in comfortable, wheel-chair accessible vans, for medical/dental & personal appointments (Weekdays during regular business hours excluding holidays)
• Professional staff-led activities such as exercises, music, games & offsite options
• Bedside care for up to 3 days for temporary/minor illness per month
• Monitoring, ordering, storage and timely distribution of MD ordered medications*
• Observation and supervision for changes in physical, mental, emotional and social functioning*
• Lodging and use of all lounge, entertainment and other common facilities and grounds at Lincoln Glen Manor and Assisted Living Center

*Note: Medications, observation and supervision are provided in accordance with the Quantitative Assessment of Resident Care Needs. Information on Assessment Scoring available by calling and speaking to the Administrator or Health Services Coordinator.

Additional care services

As residents age in place, additional services or assistance may become necessary. Many necessary additional services are available on an as-needed basis. This allows those residents who require additional services to pay for and receive the services they need. It also does not penalize residents that do not require additional services to pay for something they do not need.

Quantitative Assessment of Resident Care Needs is completed by the Health Services Coordinator (a licensed nurse) and will be discussed with residents and responsible parties prior to any additional charges being assessed. Optional services and charges require the signed approval of the responsible party and may include any of the following activities of daily living:

Our person first approach means we believe nothing should slow you down. Our skilled nursing community provides the support you need with freedom and flexibility.

What to expect with skilled nursing care

Often those in need of skilled nursing care require careful management or monitoring by licensed nurses of their physician prescribed medications. They may also need physical, occupational or speech therapy. They may have reached a point where they are no longer safe to be by themselves or care has become too complicated for their loved ones to safely manage. Often those requiring Skilled Nursing need social service support and carefully planned activities geared to their ability level.

Lincoln Glen Nursing Facility is a compassionate, church sponsored, not for profit Skilled Nursing Facility for seniors (65 and older) situated in a quiet and central residential neighborhood in Willow Glen. Acute care hospitals are a short distance away.

The Skilled Nursing Facility

With 59 beds, Lincoln Glen Nursing Facility is staffed 24/7 by a licensed and certified nursing staff. Rooms are private and semi-private with no more than two residents per semi-private room. Each room has a television with Dish Network Television service. Resident care is provided by licensed nurses supervising three care stations each assigned a staff of certified nursing assistants. Lincoln Glen Nursing Facility is fully staffed with professional activity and dietary staff, qualified therapists, a social service professional and all necessary support staff.
