At The Forum, we don’t believe in using a one-size-fits all approach in our long-term nursing care. We create personalized care plans for each resident, but we leave lifestyle preferences up to you. When you get up in the morning, what you eat, who visits you when — it’s always your choice.

The Health Care Center offers modern private or semi-private accommodations for your post-acute care, depending on availability.

We are open to the public and to Forum residents. We accept Medicare Part A & B, or private pay for anyone needing award-winning skilled nursing care.

Residents of our Skilled Nursing Center enjoy three mouthwatering chef-prepared meals daily with special dietary accommodations planned by our registered dietician.

Rehabilitation

After surgery, an illness or an injury, you need state-of-the-art equipment to recover. But more than that you need calm, experienced and patient therapists that can guide you through your exercise routine. And keep working with you until you master every exercise. Because it takes time, discipline, effort and lots of motivation to get better. Our multidisciplinary team of highly trained therapists provide individualized short-term rehabilitation therapies tailored to what your body needs. And an approach that’s equally tailored to your personality.

We offer a full range of rehabilitation care, including:

• Physical therapy: to strength muscles, accelerate healing, manage pain and increase range of motion
• Occupational therapy: to improve an individual’s ability to perform activities of daily living
• Speech therapy: to help improve the ability to communicate and swallow

We’re fully equipped to help residents design, develop and implement a personal health and wellness plan.

Our staff monitors diabetes, high blood pressure, cognitive impairment, hearing loss and visual impairment. Residents add their own personal goals and objectives to the plan. For residents with diet restrictions, our registered dietitian and executive chef create menus that are delicious while also addressing their health needs, including no or low sodium meals, low-calorie foods, vegetarian choices, seafood selections and low sugar desserts.
