Annandale Village is the only non-profit residential community in the entire southeastern United States that serves adults with developmental disabilities and acquired brain injuries at all levels of care throughout an individual’s lifetime.

Residential Services: Our Full Continuum of Care
Personal Care Homes

Eight beautifully appointed and maintained homes are the centerpiece of the Personal Care Homes on Annandale Village’s campus. These residences serve individuals at varying degrees of independence—from independent living, semi-independent living and a cottage level of supervision. All on-campus residences are designed to allow residents the ability to live as independently as possible, while providing assistance, when necessary, to create an environment that is comfortable, attractive, familiar and secure.

Independent Apartments on Campus

The Haas Building houses Villagers in the independent apartments. These 12 studio apartments allow residents the ability to live in their own apartment, while remaining part of the Annandale community. Villagers who live in these apartments need minimal supervision and enjoy a self-contained living/bedroom area, as well as a private kitchen and bathroom. In addition, Villagers must be able to complete all activities of daily living independently, as well as safely use kitchen appliances.

The Haas Independent apartments have upgraded amenities including stainless steel appliances, granite countertops, new flooring and a brand new common area where Villagers can socialize and relax at their leisure.

Semi-Independent Apartments on Campus

Two buildings make up the semi-independent apartments on Annandale’s campus—Lotz, the 19-room female house, and Gwinnett, the 19-room male house. Lotz and Gwinnett are occupied by Villagers who can complete most of their activities of daily living independently, but may require verbal prompts. A Direct Support Professional is on duty from 3 pm to 7:30 am. Villagers have a private bedroom and Jack-and-Jill style kitchen and bathroom. The individuals who live in this setting are expected to be able to do their own laundry, keep their room clean and wake up independently each day.

Cottages on Campus

Five buildings make up the Cottage residences on Annandale’s campus—two female houses that house 17 Villagers total (Broyles and Logue) and three male houses that house 30 Villagers (Banks, Brockey and Rosenblum). Cottages are occupied by Villagers who require supervision and prompting, in addition to assistance with activities of daily living. Homes are staffed by Direct Support Professionals from 3 pm to 7:30 am. Villagers have the option of a private or shared bedroom and have access to dormitory-style bathrooms.

Assisted Living

For individuals experiencing a decrease in independent living skills due to aging and/or characteristics of his or her disability, Annandale’s two assisted living environments, The Amy Somers Center for Continued Care and The Keith Keadle Center for Continued Care, bridge the gap between Annandale’s Personal Care Home options and the D. Scott Hudgens Center for Skilled Nursing.

A long-term care option that provides a combination of private room housing, supportive services and health care, the assisted living centers are the ideal solution for individuals who value their independence, yet can no longer live entirely independently. Villagers who reside in the assisted living centers require assistance with activities of daily living. Working closely with families and personal physicians, our specially-trained staff develop a care plan that respects each Villager’s needs and abilities.

Skilled Nursing

The D. Scott Hudgens Center for Skilled Nursing is the only one of its kind in the southeastern United States. Only serving adults with a developmental disability or acquired brain injury, the skilled nursing center at Annandale Village provides adults facing increasing limitations due to aging or features of their disability with a care plan specific to their diagnosis. The center offers specialized person-centered care, as well as dietary focused plans, compassionate care, social and recreational activities and a wide range of therapeutic programs.

Independent Living: All In Community Living Program

Empowered by Annandale, All In offers community-based independent living for adults with mild to moderate intellectual and developmental disabilities or acquired brain injuries. Participants, who call themselves and are affectionately known as “Mavericks,” live in their own apartments or homes throughout the metro Atlanta area. They work closely with their All In coach to build their own personalized plan suiting their needs.

Day Programs

Annandale offers two day programs that provide a wide variety of structured activities, supportive instruction and opportunities for social skill-building both on our 55-acre campus and in the community. Both programs operate Monday through Friday.

Community Access Day Program

This program serves individuals with Community Access Group Funding (CAG). In 2025, we achieved a three-year CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation, which is a testament to our quality assurance and commitment to improvement. The program focuses on:

  • Inclusion and diversity within the community
  • Practicing and defining life skills
  • Contributions to the community through volunteering
  • Full access to Annandale Village’s campus and vocational training
Private Pay Day Program

Villagers can attend the private pay day program either full-time or part-time. This is a great option for families whose loved one lives at home but desire flexibility in programming. The focus includes:

  • Encouraging independent living skills through dynamic services
  • Improving communication, self-advocacy, and social-emotional growth
  • Training and dignity in work with an on-campus workshop and community vocational opportunities

Life Enrichment

Annandale’s Life Enrichment Programs focus on personal choice while recognizing the strengths, skills, interests, and abilities of the individuals we serve.

Health and Wellness / Recreation and Sports

Our recreation and wellness programs focus on maintaining or improving endurance, balance, strength and flexibility. Villagers are actively involved in gardening, swimming, sports, Special Olympics, and utilizing the state-of-the-art Justis Blackstock Fitness Center and the Robert W. Ponder Nature Trail.

Health Services

A professional team comprised of Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants and Certified Medical Assistants ensures continuity of care. We also work with Transitional Care Physicians of America to improve healthcare outcomes and navigate changes in healthcare.

Social Services

Each Villager is partnered with a case manager who advocates for them, providing an open line of communication with families, individual support sessions, budgeting support, and guidance in reaching personal goals.

Art and Music Therapy

Villagers have the opportunity to express themselves through learning and developing new talents. Music therapy sessions, creative classes, and performance ensembles like the Annandale Choir afford opportunities for expression and collaboration while improving well-being.

Leisure and Community Activities

We believe being involved in the community promotes independence. Activities include:

  • Annandale’s Greenhouse and Walking Club
  • Bowling and Special Olympics
  • Volunteer opportunities and art exhibits
  • Attending local sporting events, concerts, and movie outings
  • Skill practice in the community (safety, money management, social skills)

Respite Services

Annandale Village provides respite opportunities for short-term stays at all levels of care, assuming availability. Our respite program gives full-time caregivers a break while giving their loved ones a stimulating and supportive environment with full access to campus offerings.
