For older adults experiencing Alzheimer’s, other forms of dementia or the later stages of Parkinson’s, we deliver specialized care and support in a home—like environment that is comfortable, safe and secure. Our approach to memory care combines compassionate care with clinical excellence.

Complex Care

The medical services needed, from the comfort of home, including the proactive and responsive management of multiple chronic illnesses that higher acuity older adults require through the coordination of medical services within our own on-site coordinated system of care.

Nursing Care

We provide care for older adults with intermittent skilled nursing needs, such as wound care or catheter management. Residents with skilled nursing needs have an individualized plan of treatment delivered by a Home Health Agency under the direction of the Primary Care Physician.

Therapy

From post-operative care to acute stroke recovery, we coordinate in-home Physical, Occupational and Speech Therapy services to meet the rehabilitative and therapeutic needs of our residents.

Hospice & Palliative Care

Compassionate end-of-life care is provided for older adult residents who develop a terminal or life-limiting illness. Our care teams focus on quality of life and comfort, including pain and symptom management.

Our Communities

Small by design. Larger assisted living facilities can feel overwhelming. We keep our communities small, with six residents in each boutique home, to provide a more personalized experience and foster deeper social connections between our residents.
