Reflections Memory Care

Our Reflections Memory Care at Fellowship Square-Mesa offers a welcoming and supportive environment for seniors experiencing memory conditions. The NCCDP-certified staff is trained as Dementia Care & Alzheimer’s Care professionals and dedicated to enhancing the quality of life for all of our residents. Our attentive caretakers get to know and understand each senior so they can relate to them while providing a safe, secure environment that helps reduce the risk of wandering.

Through relationship-centered care for each senior along with family coordination, personal service plans are developed based on the level of care necessary to help each resident thrive. Many meaningful services are offered as part of the Memory Care program, including music memories, pet therapy, aromatherapy, balance and therapeutic exercises, menu plans by our kitchen manager with ‘anytime dining,’ and much more.

ALTCS Assisted Living

As a faith-based, not-for-profit organization, Fellowship Square Mesa offers ALTCS (Arizona Long Term Care System) Assisted Living at Christian Care Manor IV, serving aging seniors with significant financial needs. Since 2001, we have provided essential care to low-income seniors with medical frailties, thanks to generous donations through the Charitable Tax Credit program.

Assisted Living at Christian Care Manor IV

Our Christian Care Manor IV building offers subsidized Assisted Living, approved by the Arizona Long Term Care System (ALTCS). This program provides financial assistance to qualifying seniors who require daily support. Residents in this program benefit from:

• A spacious one-bedroom apartment with a private walk-in shower and a fully equipped kitchen.
• Personalized care and support from trained caregivers.
• Engaging community activities and wellness programs.
• Three nutritious meals per day in a communal dining setting.
• Access to transportation services, housekeeping, and laundry assistance.

Independent Living Under HUD Section 202

For seniors who are more independent but still require financial assistance, Fellowship Square Mesa also offers subsidized Independent Living under the HUD Section 202 program. This program provides:

• Affordable rent options for low-income seniors.
• Safe and secure living spaces with modern amenities.
• Social and recreational activities to encourage an active lifestyle.
• On-site support services to assist with daily needs.

Rehabilitation Services

Out-Patient Rehabilitation Therapy may include physical, occupational, or speech therapy, depending on your individual needs. These therapy options are vital for seniors who are looking to get back on their feet after an injury or illness. Our therapy team’s focus is to help seniors stay better and stronger—as they help you not only be more active but to stay active.

Seniors who may benefit from Out-Patient Therapy would include those recovering from:

• Musculoskeletal injuries and disorders
• Heart attacks and strokes
• Surgery
• Pneumonia and respiratory infections
• Car Accidents
• Falls

Types of Out-Patient Rehabilitation Therapy

Occupational Therapy: Our occupational therapy services aim to restore your independence in daily living activities—such as eating, bathing, dressing, and other at-home tasks. It also works on strengthening your arms and improving your fine motor skills.

Physical Therapy: Your physical therapy will focus on mobility, balance, and strength, and issues like chronic pain, vertigo, gait problems, and arthritis.

Speech Therapy: You may see our speech-language pathologist to help you continue your recovery from a brain injury, head or neck surgery, or to address cognitive disorders.

Recreational & Educational Programs

Recreational Programs are prioritized at our Fellowship Square-Mesa Senior Living Community, as they help our residents have the fullest and most active lifestyle possible. Our health-centered campus offers many indoor and outdoor opportunities to be active, including activity classes, walking paths, and field trips off campus. There are two fully-staffed fitness centers complemented by our Health & Wellness and Therapy programs.

Educational Programs are also an important aspect of a Senior Living Community. We believe it is imperative that the Senior Living lifestyle promotes the stimulation of sharp minds, healthy bodies, improved memory, skill-building, and a feeling of accomplishment. Ongoing education amongst our senior residents will always be prioritized so they continue to try new things, increase their self-confidence, and meet people who share their interests.

Transportation Services

Providing complimentary Transportation Services to our residents is just one way that our Fellowship Square-Mesa Senior Living Community champions the freedom and independence that seniors require. Our caring staff safely transports seniors to local places they may need to get to or chauffeurs groups to field trips for some offsite fun. Other onsite transportation services include providing escorts to and from meals or activities on campus.

Technological Support & Smart Apartments

The award-winning technological support that Fellowship Square-Mesa has set up is enabling strong communication and helping senior residents regain control of their lives. Given the placement of over 260 Alexa Smart Properties devices within the apartments on our campus and our organization’s investment in a sophisticated campus-wide Wi-Fi structure, residents don’t need to have a smartphone or Amazon account. It is included with their Independent Living residency. The smart apartments have improved daily living functions for senior residents through voice-command use of the following: adjusting the smart thermostat, programming favorite music, calling friends, turning on lights, or using our health check-in service.
