An individual’s diagnosis should not be the inhibitor but rather the foundation of potential. In many traditional mental health settings, the diagnosis prevents the individual from experiencing greater opportunities by applying and enforcing limits. Our model of care is called Milieu Therapy. It is a positive reinforcement community based on the premise that a person’s environment can affect how they heal and requires a structured group setting overseen and managed by licensed behavioral health professionals.
Assisted Living Facility
Assisted Living is for those that need care services and supervision, including behavioral services.
Our facility and staff provide a wide range of services to meet the medical and behavioral needs of each resident. Services may include toileting, bathing, dressing, eating, mobility, social interactions, behavior modification, medication management, etc.
Behavioral Health Services
Providing structure with a positive reinforcement model is a major benefit at our community facility and a proven method for improving the well-being of our residents.
At our mental health facility, structure is individualized and not an “out-of-the-box” solution. We employ psychiatric practitioners, counselors, and medical staff that have decades of experience necessary to diagnose and provide treatment that will be effective.
Our Behavioral Program
Positive reinforcement and early intervention to protect and promote dignity and respect, not reactionary or punitive. This is the premise of our behavioral program model.
Each resident comes to Heritage Lane Behavioral Assisted Living with a history of mental illness. Some of that history is recent. Through our eyes, each resident is an individual with a myriad of issues likely affecting their overall quality of life and ability to function in and outside of the community in which they live.
Our primary goal is to create a behavioral plan with psychotropic medication management by a licensed professional to enable the resident to more fully participate in the broader community.
Another Chance
A member’s quality of life and psychosocial well-being is of utmost importance. Heritage Lane has proven results in both. For some of the most difficult cases—where members have failed multiple times in other placements, have become disruptive in the communities in which they live, or have put other peers and neighbors at risk—Heritage Lane has proven results in accepting these challenges of long-term care for mentally ill patients that result in a member becoming satisfied and happy in their placement. Imagine a member that has a history of sabotaging their living situation learning to love their living situation and speak of it in a positive way.
When your members are unhappy, they call you or member services. Sometimes these phone calls are very frequent. Our structured plans build a positive reinforcement model around specific areas to help the member rely upon themselves and not on others. This results in fewer phone calls, fewer complaints, and a happier member.

Our Services for Nursing Homes
Locating placement for a patient is a big responsibility for social service directors. Nursing homes are often large communities of 100 or more short-term and long-term care patients, making the job of a social service director and patient advocate challenging.
In these larger communities, Heritage Lane understands that one mentally ill patient can create a very challenging environment for CNAs, Nurses, PTs, OTs, STs, and other disciplines who interact with the patient. Post-Acute Rehab and Long-Term Care Facilities are rarely equipped to handle and manage patients with mental health illnesses. These patients are often disruptive to the milieu and make the job of the clinical staff very frustrating because they don’t have the training or tools necessary to address the challenges.
Whether finished with rehab and unsafe to return home, or living in long-term care and creating milieu challenges, Heritage Lane is an excellent resource for step-down placement for psychiatric assisted living. We will evaluate your patient same day and with all necessary documentation will respond within 24 hours. If accepted, admission is scheduled and Heritage Lane will take responsibility for the move and ensure the patient is transported safely to Heritage Lane.
Our Services for Hospitals
As a discharge planner, locating placement for a patient is a big responsibility. The goal is to ensure a safe discharge and proper placement while mitigating the risk of a patient readmission. Heritage Lane understands this and the services we provide are a perfect fit for your needs.
We will evaluate your patient on the same day. Once admitted, individualized planning begins on day one with a behavioral health professional. The patient is seen immediately by a psychiatric practitioner where medication is reconciled, modified if necessary, and immediately administered. With the first evaluation on day one, a treatment plan is established for the patient and staff to ensure proper delivery and coordination of medical and behavioral health services.
Our practitioners are not contracted. They are employed, and because our practitioners evaluate and treat on the first day, we reduce the risk of readmissions. We have only admitted one individual to an in-patient psychiatric facility in 5 years. Our availability of resources and proven success has a significant benefit in helping reduce the possibility of re-admissions to the hospital.
Our Services for Mental Health Clinics
If your client is unable to thrive independently and also needs a structured environment that follows a specific treatment plan, Heritage Lane is an excellent recommendation.
If you are treating clients who are living in unsafe situations, have family members that are unable to provide proper care and attention, or clients that are sabotaging family relations and need a structured environment with psychiatric assisted living, care, and intervention, Heritage Lane is a great resource for you.
As a clinician, you experience the dynamic of the client’s reality, diagnosis, environment, family, psychosocial well-being, and many other factors that drive your treatment planning. You often encounter scenarios where your client is unable to progress in their current environment or where those caring for your client are no longer able to do so. Assisted Living with a behavioral service approach can be the right fit for your client and client family. Family is encouraged to participate regularly but will not have the burden of doing this alone.
For Assisted and Independent Living Communities
If you have residents that are being evicted, are no longer following community rules, or are putting other patrons at risk, Heritage Lane is an excellent option for psychiatric assisted living.
Most assisted and independent living communities provide a calm and quiet environment for the aging population. In these larger communities, Heritage Lane understands that one or many mentally ill residents can be disruptive to the rest of the community and create an unpleasant environment. Whether in activities or dining, it can often become difficult to guarantee a quiet place to live for the other patrons.
These communities are also rarely equipped to handle and manage residents with mental illnesses. When the behaviors are exposed, caring staff are placed in situations where they are addressing the behaviors without proper training or direction. This creates an undesirable and unsafe living scenario for the rest of the community and is also unsafe for the resident who has the mental illness. In our experience, when residents who exhibit mental health challenges are co-mingled with a general aging population, sometimes very bad situations can arise that really put others at risk. We will evaluate your patient same day and with all necessary documentation will respond within 24 hours. If accepted, admission is scheduled and Heritage Lane will take responsibility for the move and ensure the patient is transported safely to Heritage Lane.
