Assisted Living

Our Old Bridge, NJ, Assisted Living community provides a home-like setting where residents can enjoy social, spiritual, or physical activities without the concerns of household chores, isolation from friends, or loss of independence. We offer various sized apartments, all with a private bath and kitchenette, plus numerous services included in one monthly fee.

We are proud to offer exceptional care in our senior homes to residents who may need assistance with activities of daily living, such as bathing, dressing and medication management. Personalized assistance and care are provided by our wellness staff 24 hours a day, 7 days a week.

Care Team Members

The Assisted Living community staff is a highly-trained, compassionate team, including: Registered and Licensed Practical Nurses on site daily, Certified Nursing Assistants available 24 hours per day, an activities director and staff, and a dietician. An ordained Reformed Church (Christian) minister coordinates religious programs for varied denominations for residents who wish to partake in these services.

Residents may choose to use the services of our Medical Director, Edward Mezic, MD, who is a geriatric specialist and a board-certified pulmonologist, as their primary care physician. He visits Assisted Living residents at least once per week.

Rehabilitation

Following an acute hospital stay, individuals requiring rehabilitation will appreciate our in-patient rehabilitation floor and on-site team of board-registered physical, occupational, and speech therapists. Our therapy programs are tailored to each person’s needs, and are conducted in a large, bright, state-of-the-art gym.

Frequent team meetings and family progress reports help ensure individuals regain their independence and can return home as quickly as possible. Families are encouraged to attend therapy sessions at our Old Bridge, NJ, rehabilitation center on Thursdays of each week to observe their loved one’s progress.

Short-term SAR residents range in age from a minimum of 50 years old to elderly members of the local community. We are especially equipped to provide care for post-surgical procedures and orthopedic, cardiopulmonary and stroke-related illnesses.

Skilled Nursing

The Skilled Nursing team at Reformed Church Home takes great pride in a long history of dignified and compassionate senior care in an exceptionally maintained setting! We offer complete nursing care for long-term residents in conjunction with an award-winning staff, which has been recognized over many years for 5-star quality of care and outstanding state survey performance.

Our Skilled Nursing center can accommodate 108 individuals, with the majority being permanent, long-term care residents. Most of these residents require full assistance with normal activities of daily living, such as dressing, bathing, feeding, toileting, and/or require more complex nursing care. About one-third of the beds are dedicated to short-term stays for our sub-acute rehabilitation program.

Long-term nursing care includes many services and amenities that make life more enjoyable for our residents while providing them with the outstanding health-care services they require, including:
